All participants must register via the Online Registration Form. Please read the instructions below carefully.



 

Each participant will receive a Registration number by email after the submission of the Online Registration Form. Please do not send any payment by bank transfer without the Registration number and participant’s name included in payment details.

All fees are stated including the VAT.

* ESPD membership: To qualify for the respective fee, the membership must be valid until the end of October 2017.
** Doctor in training: To qualify for the respective fee, a written confirmation from the attending supervisor/head of the department in question must be submitted during the online registration as a PDF attachment. No age limitation applies in this case, however the candidate shouldn’t have obtained any kind of Board Certification. 
** Student: To qualify for the respective fee, the official proof of student status must be submitted during the online registration as a PDF attachment.


Please make sure to pay your registration fee within the time frame for the registration type applicable at the date of your registration. If your payment is not received before the deadline the latter fee will be automatically charged. Unpaid registration will not be considered valid until it is fully paid and confirmed.

ESPD membership guarantees a substantial reduction from the registration fee. For more information about becoming an ESPD member/renewing your membership, individual member categories and member benefits can be found under the respective bookmarks of the ESPD official website

Important deadlines:

Early Bird Registration: until 30 June 2017
Late Registration: from 1 July 2017 until 1 October 2017
Onsite Registration: as of 2 October 2017

Delegate fee: Admission to scientific sessions and to exhibition

  Lunch IS NOT included in the registration fees

INSURANCE

The registration fee does not include participant’s insurance against accidents, sickness, cancellation, theft, property damage or loss. Participants are advised to take out adequate personal insurance.
 

CONFIRMATION

After the completion of the Online Registration Form each participant will receive via email the registration overview and a notification confirming the received payment and eventually stating the amount outstanding.
 

PAYMENT

All fees must be paid in EUR (€), free of all bank charges.

Please do not send any payment by bank transfer without the Registration number and participant’s name included in payment details.
The payment by bank transfer is available until 18 September 2017. After this date, all payments can be made only by credit/debit card.

  • Online payment details

In case of rejected online credit/debit card payment, the delegate is advised to log into the Online Registration Form and try to process the registration and the payment again. If the trouble with the payment persists, the participant is advised to contact the credit/debit card issuer/bank first and check whether the online transactions with a merchant who is based in the Czech Republic are enabled on the credit/debit card. Afterwards, the registration can be processed again.Online Payment by Credit/Debit Card

Online payments service provider: GP webpay.  

Accepted Credit / Debit Cards:                 

All online payments are processed via GP webpay secure connection. GP webpay conforms to international standards and it meets the strictest safety requirements of the MasterCard SecureCode and Verified by VISA standards as defined by the MasterCard and VISA card associations. FINAL INVOICE

The final invoice will be provided to the participants only upon request, the Secretariat will not send this document automatically. Please contact us to ask for your receipt.
The final invoice will be issued based on details provided while registering (step Contact/Invoice details). Any change of such details (address, VAT number etc.) is possible only within 3 days from the date of registration. Later changes will not be possible.
 

CANCELLATION / REFUND POLICY

  • Cancellation of Registration

Until 10 August 2017 – 50 EUR administrative fees will be deducted from all refunds
From 11 August 2017 until 20 September 2017 –50% of the fee paid will be refunded
From 21 September 2017 – no refund can be processed under any circumstances
 
All cancellations must be notified in writing (by mail, fax or email) to the Meeting Secretariat
Any post-meeting queries (e.g. requests for refunds) can be made until 20 November 2017 only. Later requests will not be considered.
For any change of name a fee of 30 EUR will be charged.
 

GROUP REGISTRATIONS

Group registrations cannot be processed online under one email address identification; each participant needs to have his/her own profile set up with unique email address.
When registering more than one person under one company identity/invoice, please contact the Meeting Secretariat to determine the preferred invoicing setting.
 

CONSENT TO THE PROCESSING OF PERSONAL DATA

  • Data Protection Directive

Officially Directive 95/46/EC on the protection of individuals with regard to the processing of personal data and on the free movement of such data.
I agree to the collection, preservation and processing of personal data contained in this form, by the administrator, C-IN, Prague Congress Centre, 5. května 65, Prague 4, 140 21 (or its employees) for the purpose stated below.
This consent is granted for all of the information contained in this form, and throughout the five years from the date of consent. At the same time I am aware of their rights under § 12 and 21 of Act No. 101/2000 Coll. on the protection of personal data.
I am familiar with all completed parts of this form, and I declare that all information are accurate and truthful and are provided voluntarily.
  • Purpose of Processing Personal Data

In accordance with § 5 of Act No. 101/2000 Coll. on the protection of personal data, all information given on this form are collected and processed solely for purposes of processing registration and accommodation requirements for the ESPD 2017 and for sending related messages to the data subject via electronic means pursuant to Act No. 480/2004 Coll. Summarized data from this form may be used by the administrator for internal statistical purposes and for internal use of the administrator.

  • Statement by Administrator

Data administrator declares that they will collect personal data to the extent necessary to accomplish the intended purpose and may be processed only in accordance with the purpose for which they were collected. Employees of the administrator or other individuals who process personal data under contract with the administrator and other persons are obliged to maintain the confidentiality of personal data, even after termination of employment or work.

RESPONSIBILITY

The participant acknowledges that he/she has no right to lodge damage claims against the organisers should the holding of the event be hindered or prevented by unexpected political or economic events or generally by force majeure, or should the non-appearance of speakers or other reasons necessitate programme changes. With registration, the participant accepts this proviso.
